The role is in a Prevention of Admissions Team 
Prevention of admission services in Hertfordshire provide acute and community-based care to help patients avoid unnecessary hospital stays.  
Key services include: 

• Hospital at Home / Virtual Wards 

• Urgent Community Response / Rapid Response 

• Hertfordshire Admission Avoidance
